The item is surprisingly sophisticated and emulates a real instrument. You can pluck individual strings, save presets for chords, and strum chords. I'm not a musician, so I used it to play random notes in the background while my friends did their best to focus on fishing. But some players have gotten so good at playing guitar in the game that they perform acoustic versions of real songs in fishing web and share clips online. All in all, it's great to see people using a single item so much in the game. The creator, Lamedeveloper, went the extra mile to create such a complex single element. (I would have loved to run around and make random strumming sounds!) The guitar has become so popular that one person created software that uses the instrument's MIDI audio files to make it easier to replicate the sound, without needing to play it directly in the game using the somewhat clunky user interface.
